Federal Card Instructions
"Thank you for taking the time to fill out your child's Federal
Card. By filling out this card you help Onslow County Schools
obtain funding which in turn is used to help meet your child's
Keep in mind that all information entered on this card must
reflect what was happening September 24, 2013.
You will only fill out two sections of this card:
1. The Student information section.
2. ONLY ONE of the next three sections (A
or B or C)
If Section A applies to you, fill it out.
If Section A does not apply, but Section B does, fill it
If neither Sections A or B apply, fill out Section C.
Wherever "Parent" is used, it refers to: Father, Mother,
Stepfather, Stepmother, Legal Guardian, or person standing in Loco
Fill out the Student Information Section in its entirety. If any
of this information has already been filled in for you, please read
carefully to make certain that it is correct. Make corrections as
The student's address should be a street address and not a P.O.
Item 8 should only be checked "Yes" if your child has an
Individual Education Plan (IEP) that is in effect on September 24,
A parent or guardian MUST sign and date
the Federal Card. Sign using your full name. (Not Mr. or Mrs. and
your last name.)
Please sign and date the card on or after September 24, 2013. Do
NOT date the card earlier than September 24,
|Section A - Uniformed Services - ACTIVE DUTY
If at least one parent or legal guardian is on active duty in
the Uniformed Services, check "Yes" and fill out Section A.
Uniformed Service includes: Army, Navy, Air Force, Marines,
Coast Guard, members of the reserves on active duty by order of the
President of the United States, Public Health Commissioned Corps,
NOAA Corps, and foreign military. This applies to members stationed
anywhere in the world, but doesn't unclude retired military.
If you are a member of the reserves on active duty, please
submit a copy of your presidential orders with this card when you
return it to your child's teacher.
If both parents are on active duty, choose one parent's
information to use, and use t to complete Section A.
|Section B Employed on Federal Property - CIVILIAN
If Section A does not apply to you, and at least one parent or
legal guardian works on one of the federal properties listed in
Section B - Item 4, check "Yes" and fill out Section B.
If both parents work on federal property, choose one parent's
information to use, and use it to complete Section B.
Enter the parent's name as it is on the employers payroll
The parent or legal guardian MUST work at one of the places
listed on the form in Section B - Item 4 to be considered to be
employed on federal property.
|Section C - Non-Federally Connected
|If Sections A and B do not apply to you, then neither parent is
on active duty or works on a listed federal property, so check
"Yes" in Section C.
